I was soooo glad to se the return of the medical information sheet! I used it about 4 years ago, when I bought my sos medical bracelet and put the information sheets all over the place - anywere I could think of!
When I went back to update my sheet after a change in my meds, I was horrified to see that the page it was on had been taken down.
As a nurse myself in a coronary care unit, I had been telling many of my patients to start using it too.
Thanks so much for making this available again - its such a fabulous and simple little piece of software I dont know why no-one thought of it earlier.
And I'll start spreading the word again - after all free is free!

I've got diabetes and think this a great idea. If I am found unconscious, it might just be a life saver.
As a first aid trainer for many years I've always told people about the products and now, after heart surgery and a mini-stroke, I have my own. The consultant at A&E was very happy to see the details contained and said he wished more people carried one. I'll continue telling people.
Very interested to see your Med emergency Information sheet. Actually I fill out one very much like it to lodge with my GP and a number of other people who might be involved if I needed urgent medical help. The sheet I am talking about is issued by my church. I am one of Jehovah`s Witnesses and we are always prepared to deal with our religious beiefs about blood transfusion. I wear one of your necklaces all the time. I wonder if you are aware that you have a large client base amongst our members. We are given a small card to carry in purse or wallet but your necklaces and bracelets are much more convenient. An excellent free source to keep all details of my daughter's medical condition informed. Very useful for school and various clubs that she is involved with an informed person is an educated person. Well done I am impressed as so are the other people that I have given the sheets to thankyou very much.
Think its a very good idea and im using it as im in and out of hospital such a lot ,one thing if i could suggest which might be handy on it if you had a medication box to write in so if you are picked up unconcious the hospital know what medication you already take i suggest this as im on some very strong stuff which could be dangerous if given with others and at least medical staff would have it to hand immediatly . The Medical Information Sheet is a great idea. All schools and children's organisations should have a standard sheet like this one to use. Many of the ones given to me aren't comprehensive enough and don't ask the right questions. If they all used a standard form in a bright colour everyone would benefit from the process being streamlined.And it would take the hassle out of handwriting reams of information for the parent or teenager.
My daughter has had epilepsy, asthma and various ailments, including having an allergic reaction to an anaesthetic (we don't know which one)since she was almost 6. We bought her her first talisman many years ago. She has had a bracelet since then but now has a pendant again (she is now 40 years of age). Our grandson also has a pendant as he is also being treated for epilepsy.
